Your first visit at Carmel Plaza Dental will accomplish two things:

FIRST-you will complete a confidential health history and registration form, which you may have received in advance. These will help the doctor in establishing your health condition and allow our staff to verify your dental coverage. Please bring information on your insurance benefits and any medications that you are taking on your first visit.

SECOND-the staff and Dr. Rafail will thoroughly examine your teeth, gums, head and neck and review your oral health. A full set of x-rays of your teeth are usually needed to complete the exam. Dr. Rafail will discuss his findings and your options with you and answer any questions you may have. At the completion of your exam one on one time will be allotted and scheduled with our hygienists according to your individual needs. In order to better serve you, your appointment may be scheduled for another day so that the proper time to accomplish your personal dental hygiene needs will be met.

Here at Carmel Plaza Dental we provide nitrous oxide, sedatives, analgesics, as well as headsets for your comfort.
